CBH Dyslexia Services, owned and managed by Claire Hurley, offers the following services:

· Full diagnostic assessment reports.

· Shorter educational assessment reports.

· Specialist dyslexia tuition.

· Exam Access Arrangements (EAA).

· Consultation & Training.

At CBH Dyslexia Services, we prioritise the privacy of your information. This Privacy Policy outlines when and why we collect your personal information, how it is utilised, the limited conditions under which it may be disclosed, and the secure manner in which it is maintained.

Types of Data Collected

When a client/learner contacts CBH Dyslexia Services for assessment or specialist teaching support, various information is collected to facilitate service provision.

This information, processed in accordance with the General Data Protection Regulations (GDPR) - May 2018, requires registration with the Information Commissioner's Office (ICO). 

The types of data collected include:

• Personal identifiers and  biographical information (e.g., date of birth).
• Contact details (e.g., home  address, email address, and telephone number).
• Sensitive personal data (e.g., details of why an individual seeks an assessment, details of a disability      or specific learning difficulty, details of past support, assessment data,      and notes).
• Dates of meetings held with a  client/learner.
• Family details (e.g., details of other family members with consent).

Test papers and accompanying information with responses are securely stored. The final report is stored electronically, password-encrypted, and retained until the learner reaches the age of 24 or for six years after the contract concludes for those over 18 at the time of the contracted service. Test papers and accompanying information are safely disposed of following the assessment and report completion.

Purpose of Data Collection

The collected information is used to plan and provide an assessment or specialist teaching support for the learner/client.

Information Sharing

Once the assessment report is completed, it is emailed with an encrypted password to the requester. The assessment is not shared with a third party without written permission. Email threads related to the assessment information are deleted.

In certain circumstances, it may be necessary to share test papers with PATOSS as part of their Assessment Practicing Certificate Renewal Process.

Access to Personal Information

You are entitled to view, amend, or delete your personal information. Send your request to Claire Hurley claire@cbhdyslexiaservices.co.uk

Note that requests to delete data will be considered on a case-by-case basis.

Changes to this Privacy Notice

Reviewed on 01.11.2023, this notice will be updated as required by ICO legislation.
